Cortisol and testosterone share a competitive relationship. When cortisol is chronically elevated — through work pressure, financial stress, relationship conflict, poor recovery — testosterone is suppressed. This is not metaphor. It is biochemistry.

Oxytocin released during positive social interaction directly moderates cortisol. Accountability relationships improve exercise consistency by more than 60%. Purpose and meaning — which emerge from connection — are independently predictive of hormonal and metabolic health. In-person connection is not replaceable by digital interaction. Every man benefits from having at least one person he can be completely honest with about how he’s actually doing.

Bioidentical Hormone Therapy

When lifestyle optimization alone isn’t enough, bioidentical hormone replacement therapy delivered via subcutaneous pellets, gels, or injections provides consistent, physiologic testosterone levels. Gels and creams rely on the ability of the skin to absorb testosterone. Therefore, levels must be checked to ensure you have adequate absorption. Injections are most often given twice weekly just below the skin. Don’t worry-we will teach you the technique. 

Pellets are compounded from plant-derived testosterone, structurally identical to what your body produces. Inserted just beneath the skin in a brief in-office procedure, they release steadily over four to six months — more closely mirroring the body’s natural delivery mechanism than any other modality.

Transdermal creams and gels are a good alternative for those needing lower levels of testosterone replacement, those with fear of needles, and those without close contact with small children or pregnant women to decrease chances of transmission.

Injections are also a great alternative for testosterone replacement. Generally, a prescription is given along with instructions for twice weekly injections just under the skin to decrease large fluctuations in levels.

Labs are obtained at close intervals when finding the appropriate dose for you. Once you have been on a steady dose, labs are needed less frequently but need to be monitored 1-2 times a year while receiving testosterone prescriptions.

Step 4 – Follow-Up & Optimization

Labs at four to six weeks confirm that levels are in the optimal range. Ongoing monitoring allows us to fine-tune your protocol over time.

It matters more than most men realize. Testosterone replacement therapy is a controlled substance with real physiological effects that require careful, ongoing clinical judgment. At Peak, your care is directed by Dr. Elisa Brantly, a board-certified urologist — a physician whose specialty training is specifically focused on male reproductive health, hormones, and the organs most affected by testosterone therapy.

Many hormone clinics in our area are run by nurse practitioners operating under broad collaborative agreements. NPs can be excellent clinicians in the right setting — but testosterone management is a domain where specialty training, prescribing judgment, and the ability to recognize and manage complications genuinely matters. Urology is the field that manages prostate disease, erythrocytosis, and the downstream consequences of hormone therapy gone wrong. Having a urologist at the helm isn’t a marketing point. It’s a meaningful clinical advantage.

Supraphysiologic testosterone refers to levels that exceed the normal physiologic range — often the result of aggressive dosing protocols that prioritize how a patient feels in the short term over what is safe long term. This is a known problem at high-volume hormone clinics where protocols are standardized, follow-up labs are inconsistent, and dose adjustments are not made with sufficient precision.

When testosterone levels are driven too high, the body responds predictably: estradiol rises (causing mood instability, fluid retention, and gynecomastia), the hematocrit climbs (thickening the blood), and the natural feedback loop that governs your hormonal axis is disrupted. These aren’t rare edge cases — they are common consequences of undertrained prescribing. At Peak, dosing is individualized, monitored, and adjusted conservatively. The goal is optimization, not maximization.

Erythrocytosis is an elevation in red blood cell mass — specifically hematocrit — that is a well-documented side effect of testosterone therapy. As testosterone levels rise, the body increases red blood cell production. When hematocrit climbs too high (generally above 54%), the blood becomes viscous and the risk of serious cardiovascular events — including stroke, deep vein thrombosis, and pulmonary embolism — increases significantly.

This is not a theoretical risk. It is one of the primary safety considerations in testosterone management, and it requires routine lab monitoring to catch early. Men being treated at clinics without consistent follow-up labs may have elevated hematocrit for months without anyone addressing it. At Peak, hematocrit is monitored at every cycle — because managing testosterone safely means managing the whole picture, not just the number that makes you feel good.

Several ways. First, you are seen by a board-certified urologist — not a mid-level provider working from a standardized protocol. Second, your treatment plan is built from your labs, your symptoms, your history, and your goals — not a template. Third, we do not optimize for the highest dose you’ll tolerate. We optimize for the dose that keeps you in a healthy physiologic range with the fewest risks over the longest timeline. And fourth, we monitor. Testosterone therapy is not appropriate for men with certain prostate conditions, untreated sleep apnea, active cardiovascular disease, or a desire for future fertility (testosterone suppresses sperm production). It is also not the right answer for every man with a low-normal testosterone level — context matters enormously.

Yes — and this is a critical conversation that is often skipped at high-volume clinics. Exogenous testosterone suppresses the body’s natural production of LH and FSH, which are the hormones that drive sperm production. Men on testosterone therapy often experience significant reductions in sperm count, and some become temporarily infertile while on treatment.

If you are interested in preserving fertility, there are alternatives — including clomiphene citrate and human chorionic gonadotropin (hCG) — that can support testosterone levels without suppressing the reproductive axis. This is exactly the kind of nuanced conversation that benefits from a urologist’s training. We will always ask about your fertility goals before recommending any hormone therapy.

Most men notice improvements in energy, sleep quality, and mood within two to four weeks. Libido and sexual function often improve in the four to six week window. Changes in body composition — muscle gain, fat loss — typically become apparent at three to six months with consistent training and nutrition. Full optimization often takes one to two cycles as we dial in your dose. Patience and consistency matter; this is a long-term investment in your health, not a quick fix.

Every patient on testosterone therapy at Peak receives follow-up labs at four to six weeks after initiation of therapy to confirm levels are in the target range. We evaluate total and free testosterone, estradiol, hematocrit, PSA, and any other markers relevant to your individual history. Before each subsequent pellet insertion — typically every four to six months — a full panel is repeated. Dose adjustments are made based on your labs and how you’re feeling. No one gets re-dosed without current labs. That’s the standard of care, and it’s non-negotiable here.
