Choosing where to get injectable treatments — Botox, fillers, or other neuromodulators — is one of the most important decisions you’ll make about your aesthetic care. The industry has expanded rapidly, and with that growth has come a wide spectrum of providers ranging from highly trained physicians to undertrained or unlicensed individuals operating outside the bounds of safe practice.
The results you get, and the safety of your experience, depend enormously on who is holding that needle. Here’s how to make a smart, informed choice.
1. Understand Who Is Actually Injecting You
This is the first and most important question to ask. In Texas, injectable treatments must be performed by or under the direct supervision of a licensed physician. That means the person injecting you should either be a physician, a licensed mid-level provider (NP or PA) with proper oversight, or a registered nurse working under a physician’s supervision and within a legitimate medical practice framework.
“Medical supervision” is not a rubber stamp. It means a physician is clinically responsible for your care — not just a name on a business license somewhere across town.
Ask directly: Who will be performing my injections? What are their credentials? Who provides medical oversight for this practice?
2. Look for a Physician-Led Practice
There is a meaningful difference between a physician-led aesthetic practice and a spa or salon that has added injections to its menu. In a physician-led setting, the clinical standard of care is set by a medical doctor who understands facial anatomy, medication interactions, contraindications, and how to manage complications if they arise.
Complications from injectables are rare, but they do happen — and when they do, having a physician involved is not a detail, it’s a safeguard.
3. Ask About Training and Experience
Credentials are the starting point, not the finish line. Injectable technique is a skill that develops with training, mentorship, and volume. Ask how long your provider has been injecting, what advanced training they’ve completed, and whether they regularly perform the specific treatment you’re seeking.
A provider who can speak confidently and specifically about their training and approach — and who welcomes your questions without defensiveness — is a good sign.
4. Expect a Real Consultation
Before any needles come out, a qualified injector should sit down with you, assess your anatomy, ask about your goals and medical history, and explain their recommended approach. They should be able to tell you which product they’re using, approximately how many units you’ll need, and what results are realistic for you specifically.
Be cautious of providers who skip the consultation, pressure you toward more product than you asked for, or give you advice without ever looking carefully at your face.
5. Verify the Products Being Used
Authentic Botox, Dysport, Letybo, Xeomin, and FDA-approved fillers are purchased through licensed medical distributors. Ask whether the products being used are authentic and properly sourced. While most patients don’t think to ask this question, a reputable provider will have no hesitation answering it.
Counterfeit and improperly stored neurotoxins exist in the market and have been associated with serious adverse events. It is a reasonable and responsible question to ask.
6. Review Their Work
Before and after photos are your best window into a provider’s aesthetic sensibility and technical skill. Look for results that appear natural, balanced, and consistent across different patients. Avoid providers whose portfolio shows overfilled lips, heavy brows, or an overall frozen or unnatural appearance — unless, of course, that’s what you’re after.
Patient reviews are also valuable, particularly those that speak to the consultation experience, how concerns were handled, and whether results matched expectations.
7. Trust Your Instincts
If a provider seems rushed, dismissive of your questions, or more focused on upselling than listening, pay attention to that. A good injector is also a good communicator who respects your autonomy and takes time to ensure you feel informed and comfortable.
