What to Know Before Your First CO2 Laser Resurfacing Treatment

CO2 laser resurfacing is one of the most effective treatments we offer for improving skin texture, tone, scarring, and signs of aging — but it’s also a bigger commitment than a quick lunchtime treatment. If you’re considering your first session, a little preparation goes a long way toward a smooth experience and great results. Here’s what to know before you book.

What CO2 Laser Resurfacing Actually Does

CO2 laser resurfacing uses focused laser energy to remove damaged outer layers of skin while heating the deeper layers to stimulate new collagen production. As the skin heals, it’s replaced with smoother, tighter, more evenly toned skin. It’s particularly effective for:

  • Fine lines and wrinkles
  • Acne scarring and other textural scarring
  • Sun damage and uneven pigmentation
  • Enlarged pores
  • Overall skin laxity

Not All CO2 Treatments Are the Same

One of the most important things to understand before your first treatment is that CO2 resurfacing exists on a spectrum, from light/fractional settings with minimal downtime to deeper, more aggressive settings with more significant recovery. Your provider will recommend a treatment depth based on your specific concerns, skin type, and how much downtime you can realistically accommodate. This is a conversation worth having in detail during your consultation, since “CO2 laser” can mean very different experiences depending on the settings used.

How to Prepare in the Weeks Before

Stop certain skincare actives. You’ll typically be asked to pause retinoids, exfoliating acids, and other potentially irritating actives for about a week before treatment to reduce sensitivity.

Avoid sun exposure and tanning. Sun-tanned or sunburned skin isn’t safe to treat and increases the risk of complications like uneven pigmentation. Strict sun avoidance (and daily SPF) for at least a few weeks before treatment is essential.

Address any cold sore history. If you have a history of cold sores/herpes simplex, let your provider know — you’ll likely be prescribed a preventive antiviral medication to take before and after treatment, since the laser can trigger an outbreak.

Plan your calendar around downtime. Depending on treatment depth, you may need anywhere from a few days to two weeks before you’re comfortable being seen in public without makeup covering active healing skin. Avoid scheduling this right before major events.

Arrange help if needed. For deeper treatments, some patients prefer to have help around the house for the first day or two while managing discomfort and the initial healing process.

What Happens the Day Of

You’ll arrive with clean skin (no makeup or skincare products). Numbing cream is typically applied before treatment, and depending on the depth planned, additional comfort measures may be used. The treatment itself generally takes 30-60 minutes depending on the area treated.

Immediately afterward, skin will look and feel similar to a sunburn — red, warm, and swollen. This is expected and normal.

What Recovery Actually Looks Like

Recovery varies significantly based on treatment depth, but generally follows this pattern:

  • Days 1-3: Redness, swelling, and warmth, similar to a significant sunburn. Some oozing or weeping of the skin is normal for deeper treatments.
  • Days 3-7: Skin begins to peel and flake as the damaged outer layer sheds. This is an important phase — picking or peeling skin prematurely can lead to scarring or pigment issues, so patience here is essential.
  • Days 7-14: Redness continues to fade, and skin gradually returns to a more normal appearance, though some pinkness can persist for several weeks, especially in fair-skinned patients.

You’ll be given specific aftercare instructions, including gentle cleansing, specific ointments or moisturizers, and a strict no-sun policy during healing.

Managing Expectations

It’s important to know that CO2 resurfacing results build over time. While you’ll see some immediate improvement in tone and texture as the skin heals, the full collagen-building benefits continue to develop over the following months. Many patients also find that one treatment, while impactful, is part of a broader long-term skincare and maintenance plan rather than a single fix.

Who Should Think Twice

CO2 resurfacing isn’t right for everyone. Patients with active acne, certain skin conditions, a history of keloid scarring, or very recent sun exposure may need to address those factors first or consider an alternative treatment. Certain skin tones also require extra care in settings selection to minimize pigment-related risks. This is exactly why a thorough consultation matters before treatment.
